最后的遗愿

所有癌症专科医生总有一项吃力不讨好的任务在身:告诉病人这场仗打输了,要病人随时得准备面对死亡。对大多数癌症患者来说,其实从确诊那一刻起,心中早知时日无多了。只不过当死神一步步逼近的当儿,任谁也难以从容面对吧。

忘不了一位七十几岁的病人,苦苦地要向命运讨回更多时间,好让他能顺利地把生意交托给子女。他这一辈子一切事情全由自己一手掌管主宰,从不愿意相信身边的任何人,包括自己的子女。想起来何其悲哀。

即使躺在医院病榻上,他还在挂着处理公事。几宗生意输了给竞争对手,可以充分感受到他对自己及家人的恼怒失望。一天,他听说又有一宗生意搞砸了,一怒之下脑充血,跌在病床上断了气。

再有,就是患者一合眼,家属立刻陷入遗产纷争。我还记得自己曾频频受到律师骚扰,只因为一名年长的病患逝世后,家属为争夺老人家遗下的店屋而对簿公堂。

我也见过家属如何在病人临终前阿谀奉承努力讨好病人,全只为了在分配遗产时多分得一些好处。

在交代后事时,一些病患会难分难舍悲痛哀戚;也有一些人坦然释怀,再三反思后已能平静地接受死亡是人生必经的历程,有些甚至为自己的葬礼拟定具体规划。

有个病人对我谈起家人时这么说:“家人都很清楚我死后没什么可以留给他们的。”可是,这名中年单身妇女的侄女外甥女仍会轮流陪她来复诊。“我们感情很好,她们小时候我经常带着她们四处去。”她追忆着往事。显然这些年轻女孩都很爱这位长辈,她把深厚的亲情留了给家人。

这名妇女三年前确诊患上末期子宫癌。我正在尽最大的努力把癌症控制住,但我们彼此都知道希望很渺茫。

她并不害怕,虔诚的信仰让她深信自己死后只是意味着灵魂离开了病魔缠身的躯体。我由衷地佩服她。她是这么一个积极乐观的人,每一回走进诊室,哪怕身上满是病痛,哪怕最近甚至已必须以雨伞当拐杖一拐一拐地撑着走进来,她脸上永远挂着阳光般灿烂的笑容,让为她看诊的人也心情愉快。

这位女士不怨天尤人或自怨自艾,我从未见她想争取多活一天或者祈求减少痛苦;她活着的每一个日子,都是一个正直圆满人生的见证。

生与死的禅意,未必每一个人都能参透,但病患多少都做好了思想与心理准备。我的一个病人在她确诊患上第四期肺癌的当天傍晚,与一家人一起坐下来谈心,对家人说,能把五个子女带大,再见证五个孙儿孙女渐渐长大,她的人生已算美满幸福,她为此深为感恩。

她谈到遗嘱,把哪一部分留给家中哪个人,为什么那么分配。接着有条不紊地为年迈的姐姐做好安排,确保自己死后,姐姐的起居生活一切妥当。

至于后事,她希望一切从简。虽然在母亲去世时,她曾经安排了老和尚到母亲灵堂诵经,但自己的葬礼,她希望以庄严为重,选择火葬离世。

她事后告诉我,自己当然希望能活得越久越好,但不想不必要地延长痛苦。她必须确保子女们不会在她已经无望复原的情况下,还要以生命维持系统来延长她的生命,并为此与我讨论了签定生前遗嘱的必要性。

濒临死亡的病患,总会谈起未完成的事业或未了的梦想。其实,每一个人的生命总会有终结的一天;对癌症患者来说,不过是所剩时日比较明确罢了。我总觉得,如果我们都能好好过好每一个日子,那余下时日是多是少,或许就不再重要了。

CanHOPE, a non-profit cancer counselling & support service and hotline, is an initiative by Parkway Cancer Centre. It is a resource for information about cancer and its available screening tests and treatments for both patients and the general public. CanHOPE is manned by an experienced, knowledgeable and caring support team who have access to comprehensive information from reputable resources on a wide range of cancer topics, including treatment options for cancer. Our support team will take as much time as required to provide thorough and personalised attention. All information provided are kept confidential.

CanHOPE also runs a face-to-face cancer counselling & support service at our offices where our support team are based. The objective of this service is to address the psychosocial needs of patients and families across an entire spectrum of the disease from diagnosis to “living with cancer” to palliative care.
